Conversion Disorder (Functional Neurological Disorder)
A mental health condition where psychological stress manifests as neurological symptoms such as paralysis, blindness, seizures, or other nervous system problems that cannot be explained by medical evaluation. The symptoms are real and cause genuine distress and disability, but they are not intentionally produced.
Prevalence: 5-10 per 100,000 per year for new cases. More common in women (2:1 ratio). Accounts for 5% of neurology clinic visits.
Common Symptoms
- Weakness or paralysis in limbs (most common)
- Abnormal movements (tremors, dystonia, myoclonus)
- Psychogenic non-epileptic seizures (PNES)
- Speech symptoms (aphonia, dysarthria, stuttering)
- Visual symptoms (blindness, double vision, tunnel vision)
- Hearing loss or deafness
- Swallowing difficulties
- Gait abnormalities or difficulty walking
- Sensory symptoms (numbness, tingling, altered sensation)
- Episodes of unresponsiveness (not true unconsciousness)
- Cognitive symptoms (memory lapses, confusion)
- Symptoms inconsistent with known neurological conditions
- Symptoms may vary or change over time
- La belle indifférence (inappropriate lack of concern about symptoms - not always present)
- Symptoms often worsen with attention or stress
Risk Factors
- Female gender
- Recent significant stressor or trauma
- History of physical or sexual abuse
- Comorbid anxiety or depression
- Personality disorders
- Family history of conversion disorder or somatic symptoms
- Recent neurological illness or injury (model for symptoms)
- Lower socioeconomic status
- History of childhood illness or hospitalization
