Narcissistic Personality Disorder

A personality disorder characterized by a grandiose sense of self-importance, need for excessive admiration, and lack of empathy for others.

Prevalence: 0.5-1% of general population. Up to 6.2% in community samples. More commonly diagnosed in men (50-75% male).

Common Symptoms

  • Grandiose sense of self-importance
  • Preoccupation with fantasies of unlimited success, power, brilliance, beauty, or ideal love
  • Belief that they are 'special' and unique
  • Need for excessive admiration
  • Sense of entitlement
  • Interpersonally exploitative behavior
  • Lack of empathy
  • Envious of others or believes others are envious of them
  • Arrogant, haughty behaviors or attitudes
  • Exaggerates achievements and talents
  • Expects special favorable treatment
  • Takes advantage of others
  • Unwilling to recognize others' needs and feelings
  • Reacts to criticism with rage, shame, or humiliation
  • Difficulty maintaining healthy relationships
  • Fragile self-esteem underneath grandiosity
  • Preoccupied with how others perceive them

Risk Factors

  • Childhood overvaluation or excessive criticism
  • Genetic vulnerability
  • Parental narcissism (modeling)
  • Abuse or neglect in childhood
  • Early childhood experiences of being 'special'
  • Male gender
  • Certain personality traits in childhood
  • Cultural factors valuing individualism

Treatment Approaches

  • Schema-Focused Therapy
  • Transference-Focused Psychotherapy
  • Mentalization-Based Therapy
  • Cognitive Behavioral Therapy (limited effectiveness)
  • Psychodynamic therapy
  • Group therapy (if suitable)
  • Medication for comorbid conditions only (no medication for NPD itself)
  • Long-term therapy essential
