Selective Mutism
Consistent failure to speak in specific social situations where speaking is expected, despite speaking in other situations.
Prevalence: 0.03-1% of children. More common in girls. Often begins before age 5.
Common Symptoms
- Inability to speak in specific social situations
- Speaking normally in comfortable settings (home)
- Interferes with school or work achievement
- Impairs social communication
- Lasts at least 1 month
- Anxiety or shyness in social settings
- Freezing when expected to speak
- Using nonverbal communication (nodding, writing)
- Clinging to caregivers
- Social withdrawal
- Sensitivity to sensory stimuli
- Difficulty with eye contact
- Expressionless face
- Awkward body language
- Fear of embarrassment
